H.264 — formally ITU-T Rec. H.264 / ISO/IEC 14496-10, also called AVC (Advanced Video Coding) or MPEG-4 Part 10 — was ratified in May 2003 by a Joint Video Team of experts from ITU-T and ISO/IEC MPEG. The goal was roughly 2× the compression efficiency of MPEG-2 at equivalent quality. It succeeded dramatically: at equal bitrate an H.264 stream looked visibly better than MPEG-2, and at equal quality files were 40-50% smaller.

H.264 became the most deployed video codec in history. Every Blu-ray disc, YouTube video before VP9, every iPhone recording, every Netflix stream, every WebRTC call, every satellite TV broadcast, every security camera since 2010 — all H.264. The patent licensing pool (MPEG LA and later Via LA) became contentious: decoding is royalty-free for consumer playback, but encoding and commercial distribution require licensing. The .h264 extension refers specifically to raw NAL-unit bytestreams (rather than .mp4/.mkv containers holding H.264 inside).
